If I have the Verizon Motorola Droid Bionic, along with the unlimited data plan, do I get mobile hotspot for free or do I still have to pay a month for it?

No you will still have to pay to use the hotspot feature.
Other way is to buy an app called PDAnet or root the phone and get Barnacle.
